Filing form for improvement exams for CBSE class 12

i got 68.8% in my cbse class 12 exams of 2013. i wish to apply for improvement exams for english and maths.can anyone please tell me how can i apply for that and what is its procedure for filling of form,getting admit card.
and where can i get the forms for filling improvement?

i got 61 % but i want to appear for improvement exam for maths and physics .. will i get a report card which improvement mentioned in it .. ? also what is the exact procedure ???
