How to fill the form and change subject as private candidate?

I got compartment in math in 12 cbse board 2012. i want to give the exam again as private candidate....but i want to choose the subjects : accountancy , business, economics , english (these were the compulsory subject i had in 12 with math as optional) and physical education as optional instead of maths........so can i change math into physical education.......and the form that i got does not states subjects accountancy and physical education........hv i brought wrong form.........please tell me how can i sort out these problems? It would be nice of you if you tell me how can i fill the form online .......

First of all i would like to tell you, you cannot fill the form online for private examination.

You need to call CBSE Helpline number for the issue: 1800-180-3456.

Is the admit card filled by the board or we have to fill and send it and they write the centres on it and send it back?

All entries except Name of the Centre and Roll No. should be filled by you. You can leave the remaining.
